All band students must read, sign and turn in all forms.All forms are in PDF format.
Practice Log
All band students are required to turn in at least one hour of practice a week. Practice logs are due by 4:00PM each Friday or last day of the school week. Practice logs must be signed by a parent.
Trip Account Permission Form
This form is to designate payments from trip accounts. A parent signature is required.
Medical Release Form
Students and parents must sign this release form before they are allowed to travel with the band.
Handbook Responsibility Form
After reading the handbook, students and parents must sign this form acknowledging that they understand the rules and policies of the THS Band program.
